THE CANYWELL APPROACH
Every patient’s situation is different. We begin by understanding the person, their health goals and the support already in place. From there, an appropriate care pathway can be discussed with the patient and their care team.
SIGNATURE CARE PATHWAYS
Program names below describe proposed service pathways. Individual services are tailored after screening and confirmation of clinical and regulatory requirements.
Support after discharge may include a home assessment, medication reconciliation, review of discharge instructions, safety assessment, patient and family education, and follow-up coordination.
Recovery support may include nursing assessments, wound monitoring, medication education, mobility and safety monitoring, and communication with the surgical team.
Ongoing support for older adults and families may include scheduled clinical visits, medication review, home-safety assessment and authorized family updates.
Extended nursing coverage may be considered according to patient needs, clinician availability, shift requirements and applicable licensing rules.
Care coordination for families balancing work, life and the responsibility of supporting a loved one’s healthcare journey.
